Community Health Worker

Community Health Workers (CHWs) work with community members to determine needs for services/resources and provide case management and crisis intervention across various program areas with a focus of collaborating closely with a variety of providers of services and resources.

Board of Directors

Join our volunteer board! The board of directors of Tri Counties Community Action Partnership plays a crucial role in advancing trauma-informed principles within the context of public health case management and promoting health and wellness in rural Glenn, Colusa, and Trinity Counties.
